Reason for Referral: John is a 17 year-old Korean-American male who was taken to the Emergency Department by his parents after making suicidal statements.
Background Information: John’s childhood and part of his adolescence were unremarkable. He is the youngest of three siblings (ages thirty-seven and thirty-six); currently lives only with his parents.
John was an outstanding student. Lately he has been falling behind in his school work and his grades have decreased significantly. He did not report any significant problems in his relationships with his family members besides his mother. He also reported that he was resentful towards his mother, blaming her for giving him his looks. He stated that he has not been involved in a romantic relationship, and he denied being sexually active. He stated that he had been teased by his peers in school about his looks and he feels embarrassed by his apperance.
John grew up in a close and supportive family environment. He has not been a victim of verbal or physical abuse in his home. He stated that he has a tremendous pressure of being successful in school since his parents have highly prioritized academic achievement in him and his older siblings. His grades have dropped from A’s to C’s and F’s he feels unable to get back on track with his school performance
John has no history of mental disorders. He is currently not taking any medications, he denied the use of any type of substances such as alcohol, cigarettes or illegal drugs. He has not reported any history of major illnesses or hospitalizations.
Current Functioning: John currently feels increasingly withdrawn. He stated that he spends significant amount of time alone, or sleeping for about fourteen hours a day. His reduced appetite has caused him to loose between five to ten pounds in five months. John seemed to be extremely focus on his physical appearance lately .He also reported that he has been experiencing an increase anxiety and depression. He stated that he feels like no one will take him seriously. He reported that he was hearing voices in his head (his parents’ voices) insulting him and urging him to kill himself. He stated he had no means or plans to kill himself. The suicidal thoughts started two months ago.
Behavioral Observations: John is an unremarkable looking Korean male who was casually dressed. His height apparent to be average for his age but he seemed to be underweight. He looked neat and clean. He had no physical abnormalities noted. John seemed to be oriented in time and person. He responded to all the questions thoughtfully, he seemed to be cognitively intact. He made good eye contact and no involuntary motor movements took place during the interview. He was cooperative and calm during the entire process. Both, his speech tone and rate were appropriate for the situation. He did not seemed to be in distress.
Testing Procedures: The tests given to John were: TAT, Rorschach and Sentence Completion. John was easy to test and highly cooperative. However, John constantly wanted positive feedback to all his provided answers. At the end of the testing he was not pleasant to be with, his distress about how good he did on the testing caused discomfort on the tester.
Testing results: The results showed personality inhibition. John appeared to be conforming, submissive and introversive. On the One Sentence Completion Task John had simplistic and concrete ways to solve problems, “If he feels bad he will take a nap and wake up and feel fine”.
